It’s very common to hear about mini facelifts, but what exactly are we talking about when we say “mini facelift”?

Is it the right treatment for you?

I’m Alejandro Mazarro, a maxillofacial surgeon specialized in Facelift Surgery.

As you can imagine, as a Facelift specialist, this is a question I often encounter.

The origins of the mini facelift

Curious to know how it all started?

Let me give you a quick overview.

The minilift was first described by Saylan in 1999 in a groundbreaking publication in the Aesthetic Surgery Journal. At that time, he called it the S-Lift, short for Short Scar Facelift, because the incision was made only in front of the ear.

Through this incision, the skin is lifted in a much more limited way than in a traditional facelift, and the SMAS is folded onto itself with sutures.

Over time, other techniques were developed, the most widely known being the MACS Lift, described by Tonnard and Verpaele (The MACS-lift short scar rhytidectomy), where the traction sutures are loop-shaped. There are also techniques that simply use small plications of the SMAS (SMAS Plication).

Mini Facelift: what makes it so attractive?

A fast procedure with quick results

One of the main reasons patients opt for a mini facelift is how quickly it can be done:

  • It only takes about 2 hours
  • Fast recovery time (around 10 days)
  • No need for a hospital stay
  • Local anesthesia or sedation is enough
  • The incision is minimal, located in front of the ear

    Easy and straightforward for surgeons

    For specialists, the procedure has some great advantages too:

    • Completed quickly (around 2 hours)
    • Doesn’t require years of additional training (any qualified surgeon can perform it)
    • No need for top-tier surgical facilities
      Too good to be true?

      Let’s put it to the test… see for yourself.

      I’ll share one of the latest mini facelift cases I performed.

      It was a while ago, and I’ll explain why.

      Carla, 56 years old, came to my office feeling frustrated with her reflection.

      Her jawline had lost definition, and her expression seemed tired. She didn’t recognize herself in the mirror and wanted to regain that refreshed, youthful look.

      However, she was clear about what she wanted: something quick, simple, and minimally invasive.

      A mini facelift seemed like the ideal solution.

      It was a procedure I had performed dozens of times, and I considered it fairly straightforward (by this time, I was already well-trained in Deep Plane Lifting, so a mini facelift felt like a breeze!).

      Just 8 days post-surgery, Carla was beyond thrilled with the results.

      And so was I.

      Her jawline was beautifully sculpted, there was barely any bruising, no pain, and she was already back at work.

      But then, frustration is probably the best way to describe the situation.

      For both of us.

      What do you think about the result now?

      A year later, Carla was almost back to where she started.

      Although her earlobes were perfectly shaped right after the surgery, the skin’s natural elasticity caused them to shift over time.

      This phenomenon, known as “pixie ear”, is a common issue in mini facelifts.

        Now, imagine being in my shoes. What would you have done?

        I’d be lying if I said I made the decision in that very moment.

        But, the results just 8 days after Carla’s surgery had brought me many new patients.

        And they all asked for the same thing: a mini facelift.

        But, despite getting what they thought they wanted, they were never 100% satisfied (and neither was I).

        Because deep down, despite asking for a mini facelift, they all wanted the same thing you do:


        To rejuvenate naturally

        Without looking overly stretched

        So, after a few days of reflection, the decision became clear.

        I haven’t performed mini facelifts in years.

        The results are stunning at first.
        But they fade pretty quickly.
        Would you go through surgery just to look the same after a year?

        The scars may seem barely noticeable at first.
        But the tension in the skin will eventually show.
        Then come the “pixie ears” and hypertrophic scars.

        I won’t lie to you: the price is unbeatable…
        But you might be in for a shock when you see the cost of corrective surgery.
        I see secondary cases every day, patients coming from all over Spain. And at that point, the solution is much more complicated: the results are never as good as with a primary procedure. These surgeries take longer, are more complex, and, of course, are far more expensive.

        Elena was 44 years old when she came to my office.
