It was described by Hamra in 1990 in his article “The Deep Plane Rhytidectomy”, although it has evolved over time, progressing through the Extended Deep Plane Facelift to reach the Preservation Facelift.

Because it works on the deeper layers, repositioning tissues back where they belong instead of just stretching them.

Traditional facelift techniques (including SMAS-based approaches) primarily address the more superficial layers of the face, often failing to correct the underlying sagging caused by descent of deep structural tissues.

By working at a deeper anatomical level, the Deep Plane technique doesn’t rely on forcefully tightening tissues but rather on repositioning them, achieving a significantly more natural outcome while simultaneously reducing the risk of hypertrophic scarring.

What Does the Deep Plane Facelift Technique Involve?

It involves releasing and repositioning facial soft tissues to restore them to their proper place.

Unlike conventional techniques, the Deep Plane Facelift represents a fundamental shift in philosophy: it aims not to stretch tissues, but to reposition facial structures to their original anatomical placement, typically without requiring additional volume with fillers.

This even more advanced variant achieves the same spectacular results in terms of naturalness and longevity (or even better), but with a notably faster and more comfortable recovery.

    How is Deep Plane Facelift recovery?

    It’s much more manageable than with traditional techniques: by minimizing subcutaneous dissection, swelling and bruising tend to be less.

    While one might assume recovery is more challenging than with traditional facelifts, the Deep Plane technique typically involves a very manageable recovery process because it’s anatomically more respectful.

    In fact, the subcutaneous dissection (which is responsible for most swelling and bruising) is much more limited than in traditional facelift techniques.

    What are the risks of a Deep Plane Facelift?

    Although in expert hands it’s a safe procedure, it’s not without risks and contraindications.

    For your safety, you should be aware of:

    • Medical contraindications: patients with serious or uncontrolled conditions (heart problems, clotting disorders, immunosuppression) shouldn’t take the risk of undergoing surgery unless it’s necessary.
    • Risk factors: smoking and poor blood pressure control significantly increase the risk of healing complications. It’s essential to stop smoking weeks before surgery and maintain proper blood pressure control.
    • However, the main risk of this surgery is putting yourself in inexperienced hands. A facelift requires precision and deep anatomical knowledge to achieve satisfactory results and prevent complications such as an artificial appearance, bleeding, keloids, or facial paralysis.

    Frequently asked questions about Deep Plane facelift

    How can I know what results I will achieve?

    I’m going to make it very easy for you.

    You’ll only need a mirror to know which technique is most appropriate for you.

    With a SMAS lift (traditional), effective traction is performed on the most posterior part of the face, so you’ll achieve what you see in the mirror if you pull upward and backward placing your fingers on the red points, just in front of the ear.

    However, with a Deep Plane lift, traction is performed from the retaining ligaments, which are much more anterior, so the expected result will be what you achieve when pulling upward (and backward) from the blue points, at the cheekbone and mandibular angle level.

    How will Deep Plane Facelift affect my facial expression? Will it look natural when I speak or smile?

    This is one of the greatest advantages of the Deep Plane technique. Your expression will remain completely intact and natural.

    By working on a deep anatomical plane and repositioning tissues without placing tension on the superficial skin, the technique completely preserves muscle mobility. You’ll be able to gesture, smile, and express yourself with absolute normality, without that “frozen” or “mask-like” effect produced by other outdated techniques.

    Does the Deep Plane Facelift also rejuvenate the neck?

    Yes, comprehensively, through a deep neck lift.

    The Deep Plane is one of the most effective techniques for rejuvenating the neck, as it allows access to and repair of the deep structures that cause its sagging, such as the platysma muscle or submandibular glands.

    By repositioning these tissues to their original place, we achieve:

    • Elimination or significant improvement of neck “bands” or “cords”
    • Redefinition of the jawline, creating a younger and more defined profile
    • Treatment of skin laxity in the area naturally, without excessive tension

    Will I need follow-up appointments after surgery?

    Postoperative follow-up is a fundamental part of the process for me, and it’s included in the surgical plan. My team and I will ensure your recovery progresses correctly.

    We’ll conduct an initial check-up the next day to remove the bandage and make sure everything is perfect. You’ll then be able to go home without bandages or drains.

    Generally, we’ll have another check during the first week and another at around 10 days, when we’ll remove the stitches.

    Traveling from outside Barcelona? I have everything perfectly organized so this isn’t an obstacle. The 10-day follow-up to remove stitches is in person, but many subsequent check-ups can be conducted via video call for your maximum convenience. Additionally, I’ll provide direct contact information to resolve any urgent questions that may arise.

    When can I return to normal life and exercise?

    Normal social life usually resumes within 15-20 days. For physical exercise, we recommend starting to walk from the first day and waiting 4 weeks for moderate activities. Everything depends on your previous health status and individual recovery. Don’t worry—we’ll provide personalized guidelines.

    Is the Deep Plane Facelift suitable for any age?

    I often receive inquiries from patients who, based on age, request information about a minilift thinking it will be sufficient, but a minilift cannot correct nasolabial folds, jowls, and even less so the neck.

    The Deep Plane Facelift is the most appropriate technique for achieving natural rejuvenation, regardless of age. That said, in younger patients, a more limited approach is usually sufficient.

    How is postoperative pain managed? What medication is prescribed? dolor postoperatorio? ¿Qué medicación se receta?

    One of the great advantages of the Deep Plane Facelift is that, being an anatomically very respectful technique, the postoperative period is notably more comfortable than people imagine.

    In fact, with my Preservation Facelift protocol, pain is currently managed with Paracetamol alone.

    This is possible because the technique avoids aggressive subcutaneous dissection, which is the main cause of pain and inflammation in facelift surgery.

    What type of anesthesia is used for a Deep Plane Facelift?
