Deep Plane Facelift Risks: A Complete Guide Before Your Surgery

What Exactly Is the Deep Plane Facelift — and Why Does It Matter for Understanding Risks?
The Deep Plane Facelift is a facial rejuvenation technique in which dissection is performed beneath the muscle (SMAS), releasing retaining ligaments — such as the zygomatic ligament — to mobilize the skin, fat, and muscle as a single unit.
This sets it apart from more superficial techniques, like SMAS plication, where the work is done above the muscle.
This technical difference is the key to understanding everything that follows: both its advantages and its real risks.
The Real Risks of Deep Plane (And Why They Are Lower Than You Think)
1. Facial Nerve Injury: The Myth That Needs to Be Debunked
For years, it was said that the Deep Plane technique carried a higher risk of facial nerve injury because it works in a deeper plane.
It’s actually the opposite.
By operating beneath the muscle, the surgeon can directly visualize the nerve structures and actively protect them — rather than working blindly in a more superficial plane, as occurs in SMAS-type techniques.
The scientific literature supports this. Recent reviews place the risk of permanent nerve injury in facelift surgery at below 0.1%.
In my experience, this misconception is still used — consciously or unconsciously — by surgeons who do not master the technique as a justification for not offering it.
2. Hematoma and Seroma: Why Deep Plane Reduces the Risk
One of the lesser-known benefits of the Deep Plane Preservation variant is highly technical but easy to understand: by elevating the skin and muscle as a single unit, much less “dead space” is created than in a SMAS facelift, where the skin is dissected independently and more extensively.
Less dead space means less fluid accumulation, which translates to:
- Lower risk of hematoma
- Lower risk of seroma
- Less postoperative swelling
- Faster and more predictable recovery
This doesn’t mean the risk is zero — hematoma remains the most common complication in any facelift and must be managed promptly if it occurs. But when performed correctly, the Deep Plane technique tends to perform better in this regard than superficial techniques.
3. Scarring and Recovery
The Risk No One Talks About: The “Fake” Deep Plane”
Here lies, in my opinion, the most serious and least discussed problem surrounding this technique.
The term “Deep Plane” has become trendy. It has become a commercial buzzword.
And this has created a worrying phenomenon: surgeons who claim to perform Deep Plane facelifts without truly mastering or executing the technique.
In my practice, I see two scenarios all too frequently:
- Patients operated on elsewhere, supposedly with Deep Plane, who come with highly visible and poorly placed scars — something that should not happen with a well-executed technique.
- Cases in the operating room during revision surgery, where I can confirm that the ligaments that should be released in a genuine Deep Plane — such as the zygomatic ligament — were never released. In other words, it was sold as Deep Plane, but what was actually performed was a superficial technique.
The Case of M., 56 Years Old: A Deep Plane Facelift That Was NOT Deep Plane
M. came to my consultation after undergoing what she was told was a “Deep Plane facelift” a few months earlier.
She was not satisfied with the result — and rightly so: she had developed a deformity that suggested muscle damage.
As you can imagine, any secondary case is more complex due to previous surgery. However, when a “ventures” into working partially in the deep plane without sufficient experience, the damage can be significant.
And that is precisely what I found in M.’s case: The zygomatic ligament had not been released (so she had effectively not undergone a Deep Plane facelift, despite what she had been told). The visible deformity when speaking was due to disinsertion of the facial mimic muscles, which I had to reconstruct. In addition to that, to compensate for the lack of deep tension, so much skin had been removed that a very evident skin retraction occurred when starting the surgery (a challenge to close again with imperceptible scars).
Were these complications of a Deep Plane facelift?
No. This was the result of a deception.
