How do you tell what’s real—and what’s just marketing?

I’m Alejandro Mazarro, an oral and maxillofacial surgeon who focuses exclusively on Deep Plane Facelift.

Over the last 10 years, I’ve operated on more than 500 patients and I’ve also performed dozens of revision surgeries to correct the consequences of poorly executed facelifts.

I’m not going to tell you I’m “the best”.

No one is.

Instead, let me go through what you should look for when choosing your surgeon—so you can decide who is truly right for you.

How to identify the right facelift surgeon: 6 criteria to consider

1) Training: Plastic Surgery or Maxillofacial Surgery?

Here’s an uncomfortable truth: not everyone performing facelifts has the most appropriate background for it.

Your surgeon’s core training matters.

Plastic Surgeon

A plastic surgeon completes 5 years of specialist training covering the entire body, from head to toe. Facial aesthetic surgery is part of that training—but only a small part.

Oral & Maxillofacial Surgeon

A maxillofacial surgeon also completes 5 years of specialist training, but focused specifically on the head and neck. That means a deep, day-to-day familiarity with facial anatomy—especially the facial nerve, which is critical in facelift surgery. And throughout their training, they operate on faces for trauma, oncology and other conditions, with one goal: to heal while leaving the least possible trace.

Does that automatically mean a maxillofacial surgeon is always the better choice for a facelift? Not necessarily.

Experience still matters most.

But it does mean they start with a far more detailed anatomical “map” of the face than most other specialists.

2) Surgical volume: how many facelifts do they perform each year?

I see many websites claiming that facelift surgery is their most common procedure.

Yet in the same breath they also offer:

breast augmentation
tummy tucks
liposuction
rhinoplasty
…

For me, that’s a red flag.

Would you rather be operated on by someone who performs one facelift a month—or one to two every week?

3) True specialisation

A facelift—especially a Deep Plane facelift—can’t be learned in a weekend course. It requires a long learning curve: typically 50 to 100 cases.

Personally, I’ve performed more than 500. And I still learn something all the time—because I’m relentless about getting better. That level of focus simply isn’t possible if you’re also doing rhinoplasties and multiple other procedures every week.

4) Surgical technique

Imagine an unmade bed.

A superficial facelift is like pulling the duvet tight while the sheets underneath stay creased.

A deep facelift tightens the sheets first—then lays the duvet neatly on top.

Why does this matter to you?

Because both the quality of the result and how long it lasts depend heavily on whether your surgeon uses a superficial technique (easier, but often less natural and less durable) or a deep technique (more demanding, but typically more natural and longer-lasting).

Now the key question: does everyone who says they do a “deep lift” actually perform a true Deep Plane facelift?

Unfortunately, no.

Many surgeons have started using the term because it’s become fashionable—but they don’t yet have the experience to do a genuine Deep Plane. That’s why you’ll often see phrases like “Mini Deep Plane facelift”: in many cases, it simply means they’re at the beginning of their learning curve.

5) Published results: marketing—or real proof?

This is where things get interesting.

How can you tell?

Simple: by looking at dozens of results.

It’s easy to show one or two impressive cases. And with a bit of “creative” photography, it’s easy to mislead: in the before photo the patient drops their chin slightly; in the after photo they lift it—and voilà, it looks like magic.

Proper before-and-after photos must be standardised: same position, same lighting, same camera. And not only in flattering angles. You should also see results with the chin down—because that’s where the quality of neck work really shows.

No before-and-after photos on the website—just beautiful text?

Be cautious.

Red flags:

❌ No cases shown (or only 1–2)
❌ Different lighting or head position between “before” and “after”
❌ No chin-down images (where neck definition is most revealing)

Green flags:

✅ Many cases available, including follow-up over time
✅ Standardised photos: same camera, same lighting, same pose
✅ Results shown even in less flattering positions

6) International reach: who do patients choose when they can travel?

Today, more than half of my patients don’t live in Barcelona. They come specifically because they’re looking for a very particular profile:

That doesn’t automatically make it wrong—but it should be understood as an opinion, not a regulated ranking. How to protect yourself: be cautious with sponsored rankings or lists that don’t explain their methodology. Always ask to see results—not a position on a list.

A surgical consultation isn’t an act of faith. It’s a technical interview.
